This service supports individuals with on-call counseling and problem solving. Our team of experienced case managers provides immediate response for assistance due to a health or personal emergency either remotely or in-person.

Immediate Support in Emergencies:
EA support can provide immediate assistance in case of a health or personal emergency, such as a sudden illness, accident, or emotional crisis. The EA team can help connect the person with the appropriate resources and services, including medical care, counseling, and financial assistance.

Counseling and Mental Health Support
EA support can provide confidential counseling and mental health support to individuals who are experiencing stress, anxiety, depression, or other mental health concerns. The EA team can provide short-term counseling, referrals to specialized mental health services, and follow-up care to ensure that the person receives the support they need.

Work-related issues
EA support can provide guidance and support to employees who are facing work-related issues such as job stress, conflicts with colleagues, or workplace harassment. The EA team can help the person develop coping strategies, provide information on workplace policies and procedures, and refer the person to appropriate resources, such as HR or legal services.

Substance Abuse and Addiction
EA support can provide assistance to individuals who are struggling with substance abuse or addiction. The EA team can provide information on treatment options, referrals to specialized services, and ongoing support to help the person achieve and maintain sobriety. Overall, 24-hour EA support can provide valuable assistance and support to individuals in the community who are facing a range of challenges.

Daily Check-In’s
Scheduled and Unscheduled In-Person Visits may be weekly, bi-weekly, or monthly if requested from the client served and/or waiver case manager.which may include but are not limited to the following: safety/safety awareness, overall health (mental, physical, emotional, social, etc.), home management/maintenance, etc. and other areas as requested

Scheduled Reminders Calls
May include but are limited to the following: medical/dental appointments, employment/related activities, transportation reminders, medication reminders, medication motivation, daily planning need, etcScheduled and Unscheduled In-Person Visits may be weekly, bi-weekly, or monthly if requested from the client served and/or waiver case manager.
