Support for emergencies, everyday challenges and staying on track.
Immediate Support During Emergencies
24-Hour Emergency Assistance can provide immediate assistance for non-life threatening health or personal emergencies to provide counsel and connect the person with the appropriate resources and services, including medical care, counseling, and financial assistance.
Who May Benefit
24-Hour Emergency Assistance may be appropriate for individuals who live independently but need reliable access to support during unexpected situations. Eligibility is determined through the individual’s waiver program, assessed needs, and support plan.
Preparedness For The Unexpected
Calm, compassionate guidance during stressful circumstances, including assistance using individualized coping strategies and identifying appropriate next steps.
Person-Centered Support
Every individual has unique needs, challenges, and goals. Our team takes a person-centered approach, providing individualized guidance, practical problem-solving, and compassionate support tailored to each person’s circumstances. By building meaningful relationships and understanding what matters most to those we serve, we help individuals navigate unexpected situations while promoting confidence, stability, and independence.
Daily Check-Ins
Phone calls and in-person visits, daily, weekly, bi-weekly, or monthly as requested by the client and/or waiver case manager, covering areas such as safety awareness, overall health, resources support, and problem solving.
Not Covered
24-Hour Emergency Assistance is designed to provide reassurance and timely support while encouraging individuals to remain as independent as possible.
This service is not continuous supervision, routine daily staffing, or a replacement for medical care, mental health treatment, homemaking, or other ongoing support services. Instead, it works alongside an individual’s broader support network to help address unexpected needs.
